EGCG epigallocatechin-3-gallate and green tea have suprisingly few trials completed. Partly due to the difficulty confirming the quantitiy of tea ingested and wide variation of green tea extracts . A few reports have positive outcomes in breast and prostate cancer, and with radiotherapy in lung cancer. Another example is positive in its results for use with chemotherapy in bladder cancers. A 2014 trial showed short term promise in slowing prostate cancer development when combined with pomegranate, broccoli and turmeric (see references). And EGCG showed productive results as a support to radiotherapy in lung cancer, with much improved 5 year progression free survival (the size of the trial meant this was not statistically verified). Despite this, there is at best mixed evidence from several clinical studies particularly those in prostate cancer, and multiple trials report no measurable impacts.
Clinical doses are in the 400 to 800mg range daily, trials in other disease have used 1000mg. As a functional food, loose leaf japanese green teas have the highest content of active ingredients. Note that some research has shown interactions with oncology treatements like TKIs (tyrosine kinase inhibitors) such as sunitinib. As always, consult with medical professionals. The clearest overall metabolic effects of ECGC are moderation of insulin levels, particularly in patient groups with elevated levels. There are mainly mixed or pre-clinical research in other important markers of inflammation.
Preventative effects of EGCG are shown in some population data, for instance colorectal, gastric. prostate and breast cancers. A meta analysis in 2023 showed a substantial preventative effect for pre-diagnostic breast cancer survival statistics. And a tendency at least, for lower recurrence in ER+ variants (see Examples). There are similar pre-diagnostic benefits reported in prostate cancer, where suppression of early stage development phases has been shown ( see references). Evidence shows clear dose dependency, and more significant risk reductions for the highest consumers, defined for instance as 5 to 15 cups daily or over 5g tea per day. In terms of EGCG levels, supplements easily reach these levels. Where studies show no effects, its probable median consumption is simply too low.

As stated previously, treatment with single-agent erlotinib [a tyrosine kinase inhibitor] did not show any impact on oral CFS [pre-cancer lesions in high risk head and neck cancer groups] compared with the placebo group. A recent large scale meta-analysis on various dietary habits of breast cancer patients post-diagnosis included 2 studies linking consumption of green tea with significant risk reductions. Overall the effects were 26% reduced risk of any breast cancer, and in particular a significantly 44% lowered risk for early stage I and II groups
Other studies find strong dose dependency and supplements can help sustain levels over time. This research also highlights dietary intake of soy isoflavones and enterolactone (ground flaxseed) shown separately
2017 meta-analysis of prostate cancer rates and effects of green tea consumption. The results indicated a tendency to reduced risk at any dose level, but the analysis of dose dependency showed that dose and frequency were major factors. More significant reductions over were measured at 5 or more cups daily, with 19% relative risk reduction rising in approximately 3 point intervals to 44% at 15 cups daily.
So the highest consumers vs lowest consuming groups see about half the rates of prostate cancer. Its not surpising that many studies have been unable to see significant results if measuring any level of green tea consumption
Nearly 70,000 chinese women in a 6 year study for incidence of colorectal cancer and consumption of green tea. Overall a 37% risk reduction was seen in those consuming green tea, and effects were dose dependent leading to more less than half the risk in the most regular tea consuming group. Doses over 150g per month of dry green tea were the highest consuming quartile.

This trial in small cell lung cancer using EGCG vs standard drugs to ease throat inflammation and pain on swallowing bought on by radiotherapy. Consumption of an EGCG solution 10ml at 3x daily provided greater pain relief than the conventional drugs. Overall survival was not changed. The small scale of the trial means the PFS benefit cannot be classed as statistically secure.
Clinical study in 472 japanese women with stage I/II/III breast cancers reporting outcomes related to consumption of green tea. Researchers analyze both pre and post-treatment effects of tea consumption levels.
In early stage I/II pre-menopausal women those drinking at least 5 cups per day of japanese standard green tea had lower rates of pre-surgery metastasis, 17% vs 24% in those consuming 4 cups or less, showing about a one third relative risk reduction. And in a seven year follow up in all patients, relative risk of recurrence was around 40% lower in early stage I/II patients. No effects were seen in advanced breast cancers.
Clinical study during pre-cancerous changes or first emergence of prostate cancer. About 30% progress within a year. Here, 600mg daily doses of EGCG supplements were used, equivalent to at least 5 cups daily of japanese standard high grade tea.
The outcome showed a nery striking reduciton in progression, only 1 of 30 patients being diagnosed in the first year compared to 9 in the control, so 3% instead of 30% progressed to a diagnosis of localized prostate cancer. Also, some imporvement in urinary condition benign prostate hyperplasia was reported
