Initial treatment for many is with aromatase inhibitors such as letrazole often with CDK4/6 inhibitors such as palbociclib (Ibrance).
Up to a fifth of patients require chemotherapy followed by other drugs such as tamoxifen or letrozole. For this group, The PENELOPE-BÂ phase III trial in 1250 patients reports only a small tendency to risk reduction of 13% by adding palbociclib. Though 4 year survival of 84%, was lower than placebo 90%. This may be linked to high degrees of unwanted side effects, seen in most patients.

The largest global drug added on to foundational ADT prostate therapy is enzalutamide, an anti-androgen.

A 2024 study comparing overall results of adding chemo to pembrolizumab (keytruda), the worlds largest selling oncology drug, to chemotherapy . For around 3/4 of patients in the PDL-1 under 50% cancer type, the report shows risk reduction of 23%. And for the likely “responders” with higher PDL-1 there was no benefit from adding chemo, only side effects.

And lung cancers are the largest group in this real world study of over 1700 patient cases with immunotherapy. Adding ipilimumab (Yervoy) to nivolumab (Opdivo) produced similar or worse outcomes and much higher side effects https://jamanetwork.com/journals/jamaoncology/fullarticle/2809043Â Though there is evidence for instance in kidney cancer that low risk patients respond well to the dual drugs.

A large multi-year study into stage III colorectal cancers followed effects of dietary habits alongside regualr first line treatments such as chemotherapy. Those drinking 4 or more cups of caffinated coffee daily radically increased both disease free survival and overall survival times

These relative increases of well beyond 2X survival times are more than most new oncology drugs that might have been added. ( see also Green Coffee Extract)
Walnuts in particular, but tree nuts in general such as brazil nuts, pecans show substantial preventative actions in population studies for many cancer. There are additionally some meta-analysis including post-diagnostic consumption activity.
Most remarkably a 2021 study in US colorectal cancer patients showing an relative increase in Overall Survival related to CRC at anyting from 2X even 3X over an average six and a half year follow up.

Needless to say, these effects are higher than those obtained from most so called second line oncology drugs.Â
There are also strong benefits in breast cancer and prostate cancer.
A large multi-national meta-analysis of soy consumption looked at the overall survival benefits and reduced recurrence rates in breast cancers. So it includes both pre- and post-diagnostic consumption.
The findings show an average 26% reduction in recurrence for those consuming up to 60mg of soy isoflavones daily, about 200mg of typical tofu for instance. And over 40% relative increased surival ratio for advanced stage III and IV patients

It goes without saying, this data is very encouraging, and the results are better than many new oncology drugs used as second line or adjuvant therapy. See Soy Isoflavones in the SUPPLEMENT LIBRARY.
Significant advantages were also shown for enterolactones – prominent in flaxseed.
Consistent and growing evidence of a direct connection to treatment results. Higher natural levels or supplementation measurably slow down cancer activity

The direct evidence in cancer research is limited to pre-clinical studies. Some of these can be compelling in nature together with related studies in human health.
